Do I have to leave some special ports open on my firewall for NAT to work
correctly?
Not that I am aware, no.
Or does NAT never traverse iptables INPUT and OUTPUT chains?
Yes, it happens all on the iptables chains themselves.
For example:
If I have a DROP by default policy, do I have to enable some ports for NAT?
No.
