Hmmm. I just checked what I've been using for some time now. I put that
line into the POSTROUTING and not the OUTPUT chain and it works, even
when the packet is locally generated. Strange, it shouldn't work from
the specs.

That IP does reside on the linux box, on the other network interface
than the one connected with the cisco.
