Similar but a little more detail

This is a similar question to the pass through firewall setup.
Basically using netfilter I would like to do a proxy arp setup where I
have a machine with a real ip on a protected network allowed to be seen
as on the real side of the firewall.  Basic setup

real area
real.ip.here.10   FIREWALL   real.ip.here.20 and lots of 10.x.x.x

now I already have the 10.x.x.x masqed just fine.  What I can't seem to
figure out is how to get messages from the real area back to the real ip
in the protected network...  I added the real ip's to the firewall so it
would respond to the packets but it doesn't seem to forward them back.
I am slowly converting to ipchains here, so a little theory wouldn't be
a bad thing for me.   In a nutshell, I have a real ip on a protected
network that I want to make appear on the real side of the firewall.  I
